Career Applications

Career applications submitted through this website are processed by FormSubmit and emailed to admin@dpcplus.com for recruitment review. This includes your contact information, preferred role and location, résumé and any optional message. FormSubmit reports retaining submission data for 30 days; uploaded files are not retained in its submission archive. Please do not submit patient information or sensitive identification or financial details. Questions about your application or information can be sent to admin@dpcplus.com. Lead Ad Data: Information submitted through Facebook Lead Ads, including form responses and demographic data.

Career Applications

Career applications include contact information, role and location preferences, a résumé, and five working-style responses. Military service details are optional, provide context only, and are not included in questionnaire scoring. Applications are processed through FormSubmit and emailed to admin@dpcplus.com for hiring review. Please do not submit patient information or confidential records. Contact admin@dpcplus.com for assistance or to request a correction or deletion.

Optional Website Analytics

With your permission, we use Google Analytics to understand visits to our general marketing pages and improve our website. Google receives information about your browser, device and page visits and uses analytics cookies. Advertising personalization is disabled. Our analytics code does not read form responses, names or email addresses, and we exclude member access, application and clinical service pages. Analytics begins only after you choose Allow Analytics. You can decline or change your choice using Analytics Settings in the footer. Your choice is saved in this browser. Event RSVPs & Optional Pledges

We store your name, email address, party size and optional pledge to coordinate our event. Only aggregate attendance and pledge totals are public. We use browser storage to remember your response and protect updates. Submissions are stored with our website hosting service. A limited, hashed network identifier is used to restrict repeated submissions. This form does not subscribe you to marketing emails. Contact admin@dpcplus.com to request a correction or deletion. Donations are processed separately by Zeffy for the Direct Care Impact Fund under Zeffy’s privacy terms; pledges are not payments and are not automatically reconciled with donations.
